Overview
A carotid Doppler ultrasound is a painless imaging test that uses sound waves to check the blood flow inside the carotid arteries — the two large blood vessels on either side of your neck that carry blood from your heart to your brain.
The device sends high-frequency sound waves into your neck and listens for the echoes that bounce back from moving blood cells. A computer then turns those echoes into a live picture and a colour map of blood flow, so the doctor can see whether the arteries are open and clear, narrowed by a fatty build-up called plaque (atherosclerosis), or partially blocked. Medical Condition

- Mild, brief discomfort if the probe presses firmly on a tender area of the neck — this passes immediately after the probe is moved.
- In very rare cases, gentle probe pressure over a heavily diseased artery may theoretically stimulate the carotid body (a small pressure-sensitive spot near the artery), causing a brief drop in heart rate or blood pressure. This is exceedingly uncommon and trained sonographers are alert to it.
- The gel applied to the skin is water-based and washes off easily; skin irritation from it is very uncommon.
- As with any imaging test, there is a small chance the images are inconclusive and a follow-up test such as CT angiography (a detailed CT scan of the blood vessels) or MRI angiography may be needed.
Preparation & Procedure

Regarding food, drink, and medications: in most hospitals no fasting is required. However, caffeine (coffee, strong tea, energy drinks) can temporarily affect blood vessel behaviour, so some radiology departments ask patients to avoid it for a few hours beforehand — check with your facility. Continue all regular medications unless your doctor has specifically said otherwise. Smoking constricts blood vessels, so patients are usually advised to avoid smoking for several hours before the test to ensure the most accurate reading.

What happens during the procedure:
- You lie on your back on an examination couch, with your head tilted slightly to one side to expose the neck.
- The sonographer applies a clear, cool water-based gel to your neck. The gel helps the sound waves travel between the probe and your skin.
- The sonographer moves a small handheld probe (transducer) slowly along both sides of your neck.
- You will hear rushing or whooshing sounds — this is the normal audio output of the Doppler machine detecting your blood flow.
- The sonographer examines both carotid arteries and usually the vertebral arteries (blood vessels at the back of the neck) as well.
- The entire scan typically takes around 20 to 45 minutes, though this varies depending on the complexity of findings and local practice.
- The gel is wiped off at the end. You can dress and leave immediately.
Aftercare
Because a carotid Doppler ultrasound is entirely non-invasive (nothing enters or touches the inside of your body), there is no physical recovery period and no aftercare restrictions. You can eat, drink, drive, and return to all normal activities immediately after leaving the scanning room.
